The playbook provides guidance on caring for patients with social needs within a COVID-19 context. It includes information on how to assess patients’ social needs, connect them to resources, and follow up to ensure needs were met and to coordinate care as appropriate. Additional considerations are included for seniors, those with behavioral health needs, and victims of intimate partner violence.
