Guidelines for the Successful Transition of People with Behavioral Health Disorders from Jail and Prison

This set of guidelines are intended to assist behavioral health and corrections, and community corrections administrators in their ability to develop effective transition plans and to drive cross-system reentry approaches between behavioral health and criminal justice agencies. The guidelines can be used as a tool within behavioral health agencies on the state and local levels to ensure that community-based approaches respond to the multi-dimensional needs of clients who are involved in the criminal justice system and the agencies that supervise them.
