30 part-time AmeriCorps members will work in teams to improve community connections and learn how to use data analytics to improve care communication, care coordination, triage, and patient surveillance across two communities in Northern Virginia. The AmeriCorps members will increase referrals to health and social services for under/uninsured community members.
