Four full-time AmeriCorps members will build the infrastructure needed to advance health equity outcomes for underserved, medically complex children and their families receiving care at public and safety-net hospitals in New York City. The AmeriCorps members will conduct risk and market analyses to identify social determinants of health and establish referral networks with community-based organizations in target communities.
